I appreciate your help and information, but Drs are not so happy(not saying mine as he has always done one but its a lot of hassle for them especially if nothing has changed since the last application/trip) It is usually a double sided form to complete.

Can you aware them and tell them you don't want to add pre exisiting? Do you have to go through this every trip?
