Bali_Jen has the right answer. You must be a current CBA client with a letter of introduction. When we opened, they also wanted local contact address, we gave a copy of ID and details of our local partner. I have been told that Kuta (?) branch wont do it, you probably are better to go to Denpasar branch on Teku Umar.
The internet banking is quite convenient, I have had no trouble using it at home and transferring money. Statements come monthly (based on minimum balance). The ATM card is pretty useless though, we have had lots of trouble using it, but this is not really an issue because we generally go into the branch anyway.
