The investigation of the incident this week could also find/has possibly already found, that the error was related to an instruction given to the aircraft and, as a consequence, the off track issue may have arisen - in essence a problem created by personel on the ground
As an aside it is unfortunate that Garuda lost an Airbus A330 ten or so years back when Air Traffic Control in Indonesia gave the crew instructions that resulted in the aircraft hitting a large hill
