I came back from Bali two weeks ago.
Here is some info, hope it helps those planning to visit Bali.

Temple entrance fee-
- Tanoh lot 10,000/person (by ticket not donation)
parking 5,000/car
Note: you cannot enter the temple
- Besakih 10,000/person (by ticket not donation)
parking 5,000/car
Note: Locals will force you to hire a guide.
I see every visitor has a guide accommodate them in the temple.
They will ask you where you are from and give you a starting price!
I paid a relatively small fee for the guide as my driver told the locals that
I have checked things on the internet and I knew a guide is not offically needed
and it will be hard for they to push further.
Remember. you pay the guide after the tour!!
Sarong is not needed as long as your legs are covered.
No one tried to sell sarong as a must to the temple.

- Pura Batur Road entrance 10,000/car
There is no set entrance fee. You put donation in a box outside the temple.
Sarong or covered legs required.
Even you have your own sarong, girls outside the temple will try every effort to charge you for a belt rental. That belt is not required as my driver told me. I did paid 2,000 for using the belt as I don't want to waste my sightseeing time. Again, they asked where you are from then give you are starting price to negotiate.

- Airport fee/tax.
Upon departure, you need to pay 100,000/person airport fee.
It is not the 20,000/person as I read from the travel books. They must have increased the fee.
They take credit card if you are out of cash.
It is relatively expensive but I think it is better than the under the counter deal that I read from travel forums.

- Car with Driver
I used a couple of drivers during my stay.
They all charge 350,000 for a full day.
200,000 for airport pickup/dropoff for a distance within 1 to 1.5 hours drive.
100,000 for airport pickup/dropoff for a distance within 30 mins(meaning kuta, nusa dua etc)

- exchange rate was
US$1 = 8,800 at the airport
US$1 = 9,030 at ubud street
US$1 = 9,200 at kuta street
