We just stayed at the Alam Kul Kul and upgraded to a villa. We paid around US$100 per night to upgrade to a villa when booking, and I didn't think it was worth it, honestly. (The room was not that large, and we never used the garden terrace. The Alam Club privileges are nice--free internet use, free local calls, free snacks like juice/water, tea sandwiches and tarts all day--but not worth an extra US$100 per night.) I'm sure you can get a much better rate if you try to upgrade on arrival, though.
The staff is lovely, and the breakfasts are quite good. When we checked in, they noticed from my husband's passport that it was his birthday, so they made him a beautiful gourmet cake and surprised him with it the next day at breakfast. Really thoughtful.
I didn't use the Jamu Spa because I thought it was quite expensive (see treatments and prices at http://www.jamutraditionalspa.com/); I took a taxi to the Melasti Beach Resort instead (Legian location, not Tuban) and really enjoyed it. It was probably a third of the price of Jamu Spa.
