I spent a few weeks in Bali with a friend with that health problem. She knew exactly what she could & couldn't eat. No need for a card - she ordered what she knew she could safely eat & there was lots of options everywhere that were fine for her. I stressed out for yonks before our trip - had the card printed, found out how to explain her dietary problems even in Balinese - totally unnecessary.
She doesn't have coeliac severely & got by just fine as would, I imagine, most adults who are used to their dietary restrictions. Kids, of course, are a different matter.
