I don't smoke and I don't have small children either. But I can tell you - that once upon a time when my children were small, they showed more consideration and respect to their peers than the chain smoking, cancer spreading ferals that spread their 2nd hand smoke around to all the unsuspecting diners in restaurants.
We don't have a say when you light up (again, MOST people don't go outside to smoke in Bali) in restaurants - we politely move to another table. But hey, there's been MANY a time when we have moved tables and then we get laughed at and snickered at "good, they can move" - comments! Rediculous.
