Obviously not all smokers are inconsiderate of others. I've seen many who will go outside the restaurant for a smoke. But there does seem to be an element of those who would normally do the right thing and go outside that will say "I'm in Bali and the law doesn't make me so I don't give a rat's".
I don't smoke and I don't have kids, so I don't inflict either of those issues on others. I've definitely been annoyed by other peoples' kids both at home, and in Bali - and of course on the flight too. As annoying as it can be though, it's not a danger of giving you cancer.
