Not only applies to "star" hotels, it's just the ones targeted in the recent operation.
Under the regulation, hotels, restaurants, tourist attractions, houses of worship, health care institutions, schools, playgrounds, public transportation facilities including ports and airports, all government buildings, traditional markets, and entertainment venues are all designated as non-smoking areas. These places are also not allowed to sell or advertise cigarettes.
But as usual the owners of these establishments are very slow to implement the rules if at all. The article also said a fine of Rp50K or 3 mths jail, I thought it was Rp50mil or 6 mths. Either way I wouldn't risk it & be careful where & when I smoked. Doesn't affect me but my wife smokes.
