I have used Telkom Flash (which was certainly far from "flash" and IM2 Broom (which doesn't go "broom-broom")
What the big two don't tell up-front (although there in fine print) is that their "unlimited" programme actually has a download limit (in IM2's case 2gb) after which speed virtually stops.
I paid Rp.199,000 for my Smartfrem modem with a monthly charge of Rp.90,000 - and there are no download limits.
On the couple of times I have experienced severe problems, Galery staff have replaced the sim free-of-charge and reset modem.
