Apply at the Post Office, you have to pre-load them with $200 minimum, you are given 2 cards immediatley, take and leave one in Bali, you keep the other one, or leave it with someone else.
You can change the pin number on either of the cards, both the same pin or different pin numbers for each card just by going on line, that way you can control how much $$ they can access.
You can top them up on-line by BPAY.
Fee for topping up isn't great, but the exchange rate is very close to street rate.
We do this with our 'shopper', and change the pin number after each withdrawal.
Cards are valid for 3 years.
