I have used social Budaya visa on a few occasions.
Answers:-
Question 1......"Single Visit". Draw a diagonal line through "For Limited Stay Purpose".
Question 2......"For Visit Purpose", and circle "Family Visit".
You will need an Indonesian sponsor in writing together with a photocopy of your sponsor's Indonesian ID.
When you have successfully completed all paperwork the visa is normally issued seven working days after lodgement. (In your case at the Indonesian Consulate in Maroubra in Sydney).
