I wondered the same before we got there, but no they only have 3 degustation menus, the Indo one, the Tasting Menu (International food, meat and seafood included) and a Surprise menu created by the chef for you only (750,000 rp).
As to clothing we just wore summer pants and nice top. I guess its easier for gals. Though from memory some of the men were certainly not dressed to the nines, not thongs or tshirt, but casual smart. You could certainly ring just in case.
It is an experience you will not forget. The little taster you get as you sit waiting for your drink which was a parmesan ball with warm oozing cheeze and truffles was out of this world and I wanted more!
