I have used the BCA Card Centre on Jl Raya Kuta on many occasions for individual withdrawls of Rp10,000,000 and paid a fee of Rp30,000 ( Aus$4.00? ).
Naturally the domestic charge by your provider will vary, depending upon policy.
My last visit to Bali was in April 2008, so the Rp30,000 fee may have increased, but it was constant for some time before then.
