Present your credit card with photo ID. I use my International Driving Licence, but obviously passport is OK.
The BCA Card Centre is located on Jl Raya Kuta on the same side of the road as Bali Bakery and heading towards Kuta.
The whole operation is most professional as you would expect from an organisation as large as the BCA Bank.
