are you traveling with. Ring them and tell them you have a wheelchair bound person. They will arrange a transfr to their wheelchairs so you can send your with the baggage. They will also have somebody waiting at the other end.
I know this is the case with QANTAS. I assume all airlines will be the same.
I would definitely do it before flying though and they can arrange suitable seating, as mentioned front row etc. Unfortunately you can't have exit rows due to the disability, as these have more room.

Don't worry about the toilets. In most planes there is a section of the side wall which opens into the corridor which makes it easy to get in. Hard to explain but the door that opens is actually the first part of 2 doors. Ask the airline which plane you will be on. Eg 737. You could even look up the layout of the plane on the net.
