Tricia, I am sure you have told your travel agent that your friend needs special care / assistance - and you can request pre-allocation of bulkhead seats.. (front) she will be assigned her seating ahead of everyone else, and will be assisted to settle in.
it's true, the aisles and seats are getting smaller, and depending on her mobility, and the type of aircraft used, she may not have direct / nearby access to facilities.
airlines often wont allow chairs in the cabin - check this detail now with your airline..make sure the purser knows her abilities / mobilities.. this can all be done at check in.
in many cases, the front bulkhead seats are close to the loos, and thus often have curtains if she feels overly selfconscious..which she should not anyhow.. everyone will understand and extend any assistance where needed...
I hope (I'm sure) she'll have a great trip.. and tell her not to worry..
