doxyxycline also has side effects, your doctor will know your family history and prescribe correctly.You don't need to go to any special clinic the information is available to all GP's, he will have to read the latest information anyway as resistance changes constantly. Some malaria meds now have resistance in certain areas, there are several medications and certainly different costs and pre and post holiday use.
No don't do a side trip without taking medication if it is an at risk area, you can get bitten even with the most vigilant personal care.
I never take malaria prevention in Bali but we planned a trip to the Islands once and I was so ill from the doxyxycline I couldn't travel.I have taken other malaria preventitives for other countries and was absolutely fine.There is a history of psychological problems with larium, if you are at risk, contra indicated with a history of depressive illness for instance.
