At resorts & higher-end restaurants etc a service charge will be added to your bill & probably none of it gets to the people who actually do the serving. In this case, if you have had good service that you'd like to reward then quietly give your tip to the person you intend it for or leave it under your plate. Do the same at places without the added on "service tax". But if the service or food is crap then I won't tip.
In taxis, round up to the nearest 5K or 10K rupiah.
By all means leave your unused toiletries etc for the cleaning staff. It may be a good idea to tip them a few dollars on arrival so they look after you extra well. It's not necessary - up to you.
Bear in mind dennisc's info about the usual wage there - 10,000Rupiah is about $1US.
