650 U$/day for the bed in the hospital - WOW, that's really expensive, even compared to Germany!
Let's assume somebody will have to stay for a month in the hospital, the costs for bed & medial treatment can easily grow to 30,000 U$ and even more, so everybody should consider a travel insurance for a MUST!
The behaviour of your airline leaves a bad taste in my mouth, I think it would have been at least a nice gesture not to charge the upgrade for business class in case of medical evacuation.
