Here are the answer's to some of your questions. Hospital $650 US per night. That was only the bed cost. I had to have two blood tests per day that was extra and medication extra and i was in there for 7 days. We had to change our seats to business class as they were the only two seats left the next flight left in economy was the 23rd of Jan, we left on the 10th of jan and husband and son left on the 7th of jan, so I was not waiting until the 23rd. The upgrade cost $430 US DOLLARS each so that is times two.
When we got to the BMIC they wanted us to pay up front but we told them we had insurance and they asked who we told them covermore and they said no problems we deal with them all the time so we will send thru a medical report and if they send back a letter of guarantee no problems we dont have to pay the insurance company will. So they did cover this but it took phone calls and a few days. So if you were in there over night I would say you would be covering the bill and they would refund you when you get home. ( The insurance company was very good)
We paid for the upgrade on plane ourselves and are waiting for paperwork for refund to come thru, also extra nights at hotel we had to pay and then claim when get home. And all the taxi rides for my husband coming up 3 - 4 tmes a day does add up.And yes I can see it would not be very good to not have enough coverage on credit card especially after having a great shopping time in bali before hand.
