Can be organised through the Tourist Police in Jln Pantai Kuta.
A friend got one while I was there in May.
No international licence needed, no test, provided copy of passport from memory and then went to police in Kerobokan to have it issued.
And yes you take a number and wait unless you have contacts or are with someone who puts you ahead of others.
Valid for a month at a time at a cost of 300000rp per renewal.
