Firstly, Bali Belly is not contagious - it's food poisoning as a result of poorly prepared food or untreated water. Secondly, when someone has Bali Belly they are not defecating all over the place - they merely have a stomach upset that depending on the seriousness may or may not allow them to perform other tasks. Therefore, it is in no way unhygienic for a person with Bali Belly to be working in a kitchen. Indeed, it would be allowed in Australia as people have stomach complaints all the time.
In defence of Warung Enak's Certificate of Hygiene - the kitchen, and staff working within it, are the most hygienic of any place I have seen in all of Indonesia.
