Clearing the airport these days is a lot quicker than it used to be because there are so few travellers. It could be as quick as 20 minutes or as much as an hour at the worst if a number of planes arrive together I would guess.
VOA can be paid in anything including plastic cards but the exchange rate is not the best and US$ seem to be a bit quicker.
I don't think there is anywhere to buy books when you are inbound at the airport but any of the larger department stores in Kuta, Sanur, Seminyak (Matahari, Hardy, Bintang, etc) as well as the book shops will have copies.
