I wonder what charges there are by Thomas Cook to administer this facility. Buying travellers cheques and purchasing international currency has always been quite expensive thru them.
I have used my Westpac debit visa card, a number of times in Bali, so I am only accessing previously deposited funds. (Although it does actually have a $50 credit limit) There were no additional charges to withdraw from the ATM in Bali although there was about a 1 - 2% fee lost in currency conversion.
