Kutas slingshot was operating at least in 2011.(youtube) I couldn't find any reference to deaths by slingshot failure in Bali. However there are numerous references to Slingshot accidents around the world.
I don't know when Bali's slingshot closed perhaps two to three years ago.
I reckon Id remember if any AFL players were involved in its demise.
