Insurance companies are notorious for slipping out of responsibilities.
Whether the place you rent from is "registered", "licences" "authorised" or otherwise is always going to be difficult if it comes to the crunch (so to speak!)
The "insurance" that comes with the car, normally with USD excess around $1000 is also often also questionable if ever you are unfortunate enough to have to call on it.
I guess that you can ask for documentation from the company and that may help, but such paperwork isn't always legit either.
Perhaps focus on larger companies, and be prepared to pay a higher daily rate.
