I was advised by my bank to try:
CIMB Niaga
BNI
HSBC
... that was after trying BCA and all the others - it would simply spit my card back out before even asking for a PIN.
I have used BNI several times now and it's worked fine, also the last two transactions I did with BCA have worked as well.
It was something to do with Visa & Mastercard changing their policy on cards with chips. If the machine wasn't modern enough to read the chip it would just reject the transaction rather than falling back onto the magnetic stripe.
Either the ATMs have been upgraded or something else has changed because I am no longer having the issue with BCA.
