You could go straight to the top and send a polite e-mail to:

I found that the only time I could get through was to ring them early on Sunday morning. They always claimed to have 'lost' my documents.
As I had scanned the documents and saved copies on my hard disk, it was easy to send them again.
In my case, it took 6 months of phonecalls and e-mails to receive a refund for a OOL to KUL flight.
My refund took the form of a credit shell which I had to use for another AA flight within 6 months.
