Unless you are travelling on a Garuda Airlines flight from Sydney, you pay VOA on arrival after you get into the airport. (I stand to be corrected if it occurs on other Garuda flights).
As for currency, I have paid in USD (if I have any), Rupiah (if I have any) or AUD. The payment amount for other currencies are signposted at the VOA counter. I would not bother changing any funds into USD just to cover the VOA as there is no benefit in doing so as the cost for conversion makes it very uneconomical.
If you are unsure of yourself and likely to be tired or stressed, you should ask the hotel to send a vehicle to pick you up so you don't have to deal with the taxi counter. I've read some posts of late that say the former way of having the destination and price displayed no longer occurs and the counter staff just nominate a price.
