The departure tax you will pay everywhere... In most countries it's factored into the cost of your flight ticket, in Indonesia and various other countries (including Thailand last time I went) you physically pay it at the airport.

It's not a extra cost. It's just a more obvious one.

I suppose at some point it becomes prohibitive -but there's probably still a long way to go yet until you reach that point.
