I would probably do Bali > Sumatra (most likely via CGK) as I doubt there are direct flights.
Then do Sumatra via CGK to SUB. Once you're finished in Surabaya go back to Bali, or fly from Surabaya to wherever it is you are returning to if you can.
I have only been to Surabaya, not Sumatra so I can't help with where to land. I've heard Medan is nice though.
