I always say to them that I'll pay more than the going rate to take me the whole distance.
On arrival, that means through to my driver who is waiting for me and who will then take over, and on departure it means up to the check in counter. Rp50K has always gotten my bags lifted onto the belts at security screenings and at the check in counter.
When I was a bit younger and stronger I didn't mind lifting but I have neck and back issues now and it's $5 well spent as far as I'm concerned. :)
