At biggger places, the rice they use is probably pre-fried in bulk, but at smaller places it's fried fresh each dish as ordered, not left longer to get the attention of flies etc. Having said that, the rice used for Nasi Gorng is cooked the night before, thus at bigger places it's greater bulk, greater risk.

IMHO the very tastiest is from the street-side Kaki Lima, conflagrated before your eyes and nothing will live through that
