Last year for my brother's 50th birthday (8 family members went with him to Bali so it was great fun), we went to a Sanur restaurant which had some Balinese dancers. My brother loved it, especially when the dancers cheerfully posed for photos with him and brought out a sparkly dessert just for him without us even asking.
You are very spot on in saying he responds most favourably to Balinese people who are endlessly kind and helpful in helping him negotiate steps, escalators, squat-toilets (our biggest fear because of his mobility problems) etc.
The white water rafting sounds good. Where do you do that? I would not like to take any significant risks because he is on blood thinning medication however if it is gentle, it might be worth a try. He would not be able to manage more than few stairs as is takes two people to help him and he doesn't enjoy that feeling of dependency. I don't know about a massage or pedicure as he hates having his feet touched. I'll save that one for myself :)
