there is a supermarket on the right hand side in Tuban, sorry I can't remember the name, it is across the road from Circle K and has a small games arcade and some food stalls, anyway, there is a phone shop there and they charge rp5000 for a SIM with a small amount of complimentary credit. They will set it up for you and sell you any extra credit you need. It is very convenient and the credit is sold at local prices not with the added tourist tax.
