I'm lactose intolerant and really struggle in Bali dining out. I do take long life skim milk with me and not a problem declaring it on arrival in Bali (skim does contain lactose but I'm lucky I can tolerate it in small amounts)
I take 'Lacteeze' an enzyme supplement when we go out at night for dinner (Chemist Warehouse is the cheapest) and find I can enjoy most foods without the awful after effects. If I over do it... it catches up with me and I pay for it.
I have seen a few people take their own margarine/spread at breakfast and some people take their own bread and lactose free milk... I think it's the way to go. After it's used the more space/weight for the return journey.
I really sympathize with your friend's daughter with the double whammy lactose and fructose :-(
