Depending on how pressed you are for time, if he is the only one in your party diving, and he is quite keen to do Tulamben (which is a nice dive) I'd probably arrange it from here.
I haven't used any dive shops down south and can't recommend any in Kuta, but people tend to say good things about Crystal Divers in Sanur and certainly their website would give you an idea of prices. There are a number of dive shops in Sanur and they probably do pick up from Kuta. I'm sure you could get recommendations from others on this site for a dive shop in Kuta.
What you're looking for is generally called 'discover scuba' I think.
The drive to Tulamben in probably a couple of hours (maybe more, the way traffic is down Kuta way at the moment!)
