Best one is Paddys Adventure ATV (google them for website)- you can organise when you get there and barter the price down with any tour booking stall in Bali....we paid $60US a year ago.
Includes transport, lunch....
We have done this tour over 5 times and it still hasn't lost its appeal! Doing it again when we head back in two weeks time!
There is another place that does it, but the tour was not that good.
