1. Usually they don't even heat the water underneath.
2. some of the restaurants up at Kintamani (Penolokan) take uneaten food from guests plates and put it back in (know people that work in 3 up there and they say its common practice in many places).
3. It can be there all day, then if not eaten goes in the fridge, then pulled out again for the next day. My favorite eating place up the road does this and the food all tastes like the fridge the next day. I just ask to serve myself and give the stuff a whiff before I put it on my plate, they are used to my crazy ways.
