We always take a car over so ferry is the only option. Yes we have always felt safe, they are big boats but yes some are in better condition than others. We travelled with teenagers and usually go onto the top deck, take food drinks and play card games, chat to locals and just take in the scenery. Yes it is slow, between 4 to 6hours depending on how long it takes to dock as there can be many ferries waiting when you get to the port. Have also been on the ferry between lombok and sumbawa and it was the same.
Only advice would be try your best not to use the toilets, pretty yuk!!!
