a few trips ago our travel agent didn't notice that my passport was ni my maiden name and did the ticketing in my married name.
flew adel to perth to be told when checking in at perth that ticket and passport didn't match - after much failed scrambling to obtain marriage certificates etc (they wanted originals!) I had to buy a new ticket on the spot or return home :(
