Very true, orpanages are not tourist attractions and shouldn't be seen as such. Advertising works the same for any organisation, whether a multinational corporation or humanitarian non profit organiation. If no one knows about the orphanages and the good work they do, and don't visit them to find out exactly what they are about, then aquiring adequate support and funds to do their good work will be almost impossible. If the phrase orphanage tourism opens the eyes of tourists who don't know what it is to be doing it tough, like these children are, then I can live the phrase.
