Your assertion that "The Canberra & Sydney forms are quite different is open to question.
The information required is identical on both forms even down to the same numerical sequence of the answers required.
I have both forms in front of me at the present moment and the only differences I can see are as follows:-
1)Each form has a heading indicating its origin.
2)The numbers 1 through to 5 on the Sydney form are replaced by the equivalent Roman numerals on the Canberra document.
3)The format of the printed form. The Canberra form has lines of small squares after each question so that the answers may be more easily read, whereas the Sydney does not and thereby making things more difficult to keep one's answers in a straight line.
I doubt if these differences constitute a "quite different" description.
