I use a visa debit card issued by my local credit union (not a national one) and I don't get charged any fees for using ATMs in Bali.

I get very close to the going exchange rate (at reputable money changers) at the time too, so I'm not having fees built into a poor exchange rate. Most days last trip I was getting around Rp8800 when friends were getting Rp8900 for their cash exchanges.
