You should carry all your diabetic supplies on board with you including sharps - as if anything happens to your luggage, you are not left with a medical emergency. Your letter from the doctor needs to state who the person is and that they are required to carry diabetic supplies and sharps on board. We have never had a drama. Try to keep your insulin in a cool bag though and get it into a fridge as soon as you can when you arrive in Bali.
