the shop is called smartfren, its located in the row of shops next door to carrefour. The cost is 199,000 for unlimited usage for one month, you will need to provide ID for registration. I cant remember what the speed was however I use skype every day without any problems. Perhaps check their web page, just google smartfren. There is also now a shop in Jl Raya Kuta.
