Both Pro XL & Simpati now have micro sims for Iphone 4
How readily available they are in Bali I'm not sure
Have read that most places have a sim cutter available anyway
There are Apple/Iphone places in Bali where you could enquire
We have an unlocked Iphone 4 here in Australia & just cut down our existing pre paid Optus sim from a template on the internet

Optus store can provide a Micro Sim for $2 and transfer your number over (prepaid) but at the time they had none in stock, so we cut our own
We will probably do the same (cut a prepaid sim to size) on arrival in Jakarta if they are not readily available
Prepaid sim as in Australia have a data allowance
