A couple of recent recommendations which I have on my list for Legian are Spa Bali & Santai Spa. The spa at Melasti Beach Resort is also good.
If you don't mind travelling a bit then the day spa at Tanah Lot is gorgeous - if you email them they will send you a copy of their current price list
