Depending on how many of you go, you can get better deals for more people, but $45 - $50pp is around the price. We book through Johnny at the Bagus Pub in Poppies II & get reasonable prices. The normal Bali casual attire is quite suitable, but if you want to dress up a bit, you won't be out of place. You can usually book a day or 2 beforehand. Longer if you like, but not necessary.
